Integral Diagnostics (ASX: IDX) – 2024 AGM Chair and CEO Presentations

Integral Diagnostics delivered strong FY26 financial results, with revenue climbing 25.6% to $788.7 million and operating EBITDA surging 30.3% to $164.8 million. The operating EBITDA margin expanded 80 basis points to 20.9%, underpinned by operational leverage and $14 million in synergies from the Capitol Health merger completed in December 2024. Operating net profit after tax increased 50.1% to $47.4 million, and operating diluted earnings per share grew 23.6% to 12.6 cents. The company increased its final dividend 50% to 6.0 cents, bringing total dividends for the year to 9.3 cents.

The narrative shifted markedly in the Q1 FY27 trading update delivered by new Chief Executive Jason Martinez at the annual general meeting. First quarter revenue growth slowed to just 3.0%, or 4.4% when adjusting for closed sites and currency movements. In Australia, where the company generates the bulk of its earnings, revenue growth excluding closed sites reached 5.1%, trailing estimated Medicare benefits growth of 7.1%. The shortfall reflects a supply constraint. IDX controlled 11.9% of the Australian diagnostic imaging market, up from 7.1% three years earlier, with 141 clinics across Australia and New Zealand, 445 radiologists and 144 contracted teleradiologists. Radiologist and sonographer shortages prevented the company from capturing available referral volume.

Radiologist costs as a percentage of revenue deteriorated in Q1 due to a combination of factors. Higher reliance on locums to fill gaps, additional incentives paid in July and August to reduce patient lists, and the rollout of Clario reporting software in Victoria created short-term productivity headwinds. The removal of quarterly superannuation caps also pulled 52% of expected annual radiologist superannuation into the first quarter, creating a timing distortion that will partially reverse. Metro Victoria underperformed at both the top and bottom line, prompting the appointment of a new regional general manager with over 25 years of industry experience. South Coast Radiology grew revenue in line with clinician availability.

The company guided for full-year FY27 revenue growth between 4.0% and 6.0%, down from the trajectory suggested by FY26 momentum. More significantly, management expects operating EBITDA margin to contract to a range of 19.5% to 20.5%, with improvement anticipated in the second half as cost initiatives take effect. The company is implementing stronger labour forecasting and scheduling discipline, accelerating technology deployment to lift radiologist productivity, and consolidating procurement and facilities management on a national basis. The Board approved a Prioritising Access and Customer Experience program covering online booking, digital forms, electronic referrals and voice AI.

The question for shareholders centres on execution. Diagnostic imaging benefits from structural tailwinds including an ageing population, rising chronic disease prevalence, MRI deregulation and the National Lung Cancer Screening Program. IDX operates at scale in a consolidated market and has the technology foundations in place. Whether management can reverse the margin compression trajectory and extract consistent profitability improvements across its network will become clearer at the February 2027 half-year result, where the company has committed to quantifying benefits and delivery costs separately from its transformation initiatives.
