MPL (ASX: MPL) – FY26 Results and Investor Presentation

Medibank Private has reported underlying earnings per share growth of 2.9% to 23.1 cents, maintaining profitability despite an increasingly competitive private health insurance environment. What stands out is the company’s progress in building a diversified earnings base, with the Medibank Health segment contributing approximately 12% of group operating profit and surging 31.3% to $100.7 million for the year.

The health insurance core remains resilient. Resident policyholder numbers grew 1.1%, with net additions of 22.1 thousand despite elevated competition and aggregator marketing pressure. The gross margin held steady at 17.0%, while the operating expense ratio remained at 8.0%, demonstrating that Medibank can absorb competitive pressures without deteriorating profitability. This represents solid operational discipline in a sector where margin compression is a genuine risk.

The company’s capacity to maintain stable margins while growing comes from two sources. First, resident premium revenue expanded 4.5% with disciplined pricing, and second, resident claims growth of 4.4% reflected lower utilisation outside traditional hospital settings combined with improved risk equalisation. The company flagged that a $74.8 million COVID-19 utilisation benefit helped FY26, and this boost will not repeat in FY27, creating a headwind for comparison purposes.

Medibank Health represents the company’s platform play on the broader shift toward community-based and preventative care. The segment achieved revenue growth of 30.8% to $634.8 million, with the acquisition of Better Medical contributing $6.2 million of profit over six months. The primary care arm grew consultations by 26.7%, while acute home health admissions surged 13.7% to support 194,000 hospital bed days saved. These metrics speak to real demand for alternative care settings, though gross margins compressed 180 basis points to 51.5% as the company invested in the Live Better wellbeing program.

Non-resident health insurance proved challenging. Policy units contracted 2.3%, reflecting tighter migration settings and normalisation of large student cohorts acquired after border reopening. Gross profit fell 1.2%, though revenue still grew 5.0%, indicating pricing discipline despite the volume headwind. Management expects stabilisation and solid growth in FY27 as student retention initiatives take effect and worker-visa growth accelerates.

The balance sheet remains strong, with the prescribed capital amount coverage ratio lifting to 1.9x, providing capacity for the company’s M&A ambitions in primary care and wellbeing. The ordinary dividend grew 6.7% to 19.2 cents per share, fully franked, with a payout ratio of 83.0% of underlying earnings, within the stated target range of 75% to 85%.

For investors, the key question is execution on the health transition narrative. Management targets Medibank Health segment profit exceeding $200 million by FY30 and is guiding for growth of approximately 25% in FY27 from a full-year contribution of Better Medical and organic expansion. Resident PHI growth must remain disciplined yet accelerate modestly, while non-resident stabilises. The upcoming FY27 April premium increase and the company’s ability to convert health engagement into retention will be critical data points to monitor.
