Mesoblast Limited has demonstrated meaningful commercial momentum with Ryoncil, its flagship cellular medicine, delivering US$115 million in net revenue during its first full year post-FDA approval, with US$36 million generated in the fourth quarter alone. This revenue trajectory validates the commercial potential of the company’s allogeneic cellular platform in treating life-threatening inflammatory diseases.

The revenue achievement takes on additional importance when considered against Mesoblast’s disciplined approach to capital allocation. The company reported net operating cash spend of just US$43.8 million for the full year, resulting in a cash balance of US$103 million as of June 30, 2026. This financial position demonstrates that Ryoncil is generating meaningful revenue while the company maintains sufficient dry powder to fund its pipeline advancement and reach critical inflection points.

Mesoblast’s strategy to extend Ryoncil’s market opportunity appears well-positioned for success. The company has commenced a registration trial to expand the product’s label into adults with steroid-refractory acute graft versus host disease, a market the company estimates is three times larger than the current pediatric indication. The company expects to activate up to 40 sites across the United States this year, representing approximately 60 percent of the roughly 8,500 annual U.S. allogeneic adult bone marrow transplant population. This expansion alone could substantially increase the addressable market.

Beyond SR-aGvHD, Mesoblast received Investigational New Drug clearance to proceed directly to a registrational trial evaluating Ryoncil in children aged 5 to 9 years with Duchenne muscular dystrophy, which affects approximately 15,000 children in the United States. The direct path to a registrational trial, rather than earlier-stage development, signals the FDA views the clinical evidence supporting this indication as compelling.

The company’s pipeline progress extends beyond Ryoncil. Mesoblast achieved its target of treating at least 300 patients in the pivotal Phase 3 MSB-DR004 trial of rexlemestrocel-L for chronic low back pain associated with degenerative disc disease. Additionally, the company received a Biologics License Application filing number from the FDA for rexlemestrocel-L in prevention of life-threatening gastrointestinal bleeding in end-stage heart failure patients with a left ventricular assist device, representing another potential pathway to label expansion.
